Background: Therapeutic advances in the management of IBD have led to evolution of treatment targets.1Beyond clinical symptoms, the goal of therapy in Crohn’s disease is endoscopic mucosal healing, which has been associated with reduced rates of flare, hospitalisation and surgery.12However, histological inflammation persists in around one‐third of biopsies from endoscopically healed mucosa, raising the question as to whether amelioration of microscopic inflammation, or “complete remission”, may yield additional prognostic benefit beyond endoscopic mucosal healing in Crohn’s disease.
