Osteopoikilosis hiding active sacroiliitis in Psoriatic Arthritis

Background: A 45-year-old woman with diagnosis of PsA reported a 3-month history of low-back pain. At clinical evaluation, she showed elective pain of the SI joints and stiffness of the lumbar spine. Routine blood tests were normal, except for increased CRP. On pelvic X-rays, there were diffuse osteoaddensing lesions of ileum and ischio–pubic branches and femoral heads, but apparently no signs of sacroiliitis.
